Post Title: Housing Support Worker


Job Purpose and Role

  • To provide a high quality and effective housing support service which responds to the needs of care leavers, in a timely fashion
  • To work collaboratively with Social Worker’s, Young Person’s Advisors and partner agencies in order to produce the best outcomes for young people living in LAC Transition’s supported accommodation.
  • To provide care leavers with the opportunity to achieve a full range of independent living skills so that they are ready to move onto more settled and permanent accommodation

Principle duties and responsibilities

  • To meet with a young person prior to their move to supported accommodation, where possible, to establish a relationship with them and contribute to the preparation for their move.

  • To contribute to making sure that the supported accommodation is ready, safe and secure for the young person to move into. This may include undertaking some general cleaning tasks and ensuring that any necessary repairs are reported and completed to a satisfactory standard, in a timely fashion

  • To support the young person’s move to supported accommodation

  • To visit young person in their new home on a regular basis to provide the agreed weekly hours of housing support assessed to be necessary to meet their needs.

  • To closely monitor the young person’s transition to supported accommodation and to report any emerging concerns or risks to the Senior Housing Worker, Team Manager or Practice Manager

  • To work directly with young people to ascertain their wishes and feelings in respect of all plans relating to them and to encourage them to be involved in the planning process, as appropriate

  • To contribute to the development of an Individual Support Plan for each young person which is informed by the assessment of their needs completed by their Social Worker, their care and pathway plan and which sets SMART goals for the young person to work toward in the next month which will incrementally improve their independent living skills.

  • To deliver the intervention agreed within the individual support plan through direct work with the young person on an individual and group work basis

  • To support young people to purchase furniture and equipment from their Setting up Home Grant

  • To supervise how young people spend their personal allowance when they initially move into their new home to ensure that they budget effectively, pay bills and buy essential items.

  • To contribute to the assessment and review of a young person’s support needs and revise the young person’s Individual support Plan accordingly

  • To support young people to access education, work placements, employment, social activities and other appointments, as necessary.

  • To share information with Social workers, young Person’s Advisors and workers from partner agencies on a regular basis to keep them informed about the young person’s progress and inform them of any change in the young person’s circumstances.
