Are you looking for a challenging and rewarding role with career developments? Then look no further??? A new opportunity has arisen for a Patient Liaison Coordinator to join our team at HMP Stafford. HMP Stafford is a category C training prison for adult males holding up to 741 prisoners. The significant pressure that high volumes of external escort and bedwatches place on prison operators, providers and the NHS is widely known and understood. Our proposal to overcome these challenges is the introduction of a Patient Liaison Coordinator role. The purpose of the role is to balance the need to ensure all patients receive the highest levels of care, ensuring parity, whilst also seeking to reduce the operational and cost burden of escorting patients to external hospital appointments and/or during hospital admissions. About the Job: The role will seek to implement innovative solutions to avoid unnecessary hospital visits and/or reduce the requirement for patients to remain in hospital when an alternative solution is available. As the Patient Liaison Coordinator, you will: • Develop strategic relationships with senior managers from local acute hospitals to ensure effective communication mechanisms are in place and are replicated across all departments • Work with hospital trusts to share knowledge and improve understanding of how healthcare is delivered into secure environments • Overcome local barriers where some elements of consultant led care have already been delivered through Tele-Medicine without the need for repeated appointments when care is transferred to local acute trusts • To build relationships to increase the number of in-reach clinics so first appointments or follow ups can be conducted with the use of an escort. • Seek to deliver pre-op assessments on site where possible to avoid additional escorts • Will identify any trends and suitable training courses for staff to attend in order to reduce the need for escort. For example training the paramedics and nurses to be able to put on and remove POP's and combine the use of in-reach diagnostics to manage patients post fractures whilst actively using a fracture consultant via telemedicine As the Patient Liaison Coordinator you will case manage patients who are identified as requiring a large number of hospital appointments and/or admissions to facilitate the delivery of effective care, and will work in conjunction with BSMHT in their capacity to overseeing inpatient bed placements. You will also highlight any ongoing cases and barriers in returning patients back from hospital, in conjunction with the relevant Local Authorities, to ensure they meet their statutory duties under the Care Act. By taking a case management approach this will have a direct impact in providing an optimal solution to unlocking bed spaces, whilst also working to deliver a positive patient experience. Further job information: This role is a fixed-term full time contract for 12 months, working 37.5 hours per week. The salary is GBP35,000 - GBP40,000 per annum - dependent on experience.
