Purpose of job:

•To deliver a targeted Occupational Therapy service focused on the review of double up care packages for patients in receipt of NHS Continuing Healthcare funding, with the aim of identifying options to reduce the need for carer input through the use of a range of equipment and manual handling techniques.

•To support the early review of post hospital discharge packages of care to promote independence and ensure best use of limited care funds

•To provide specialist occupational therapy input and advice on requests to increase levels of care.

It is essential that the post holder has:

Detailed experience in Occupational Therapy, including assessment and working with both individuals and carers to improve their well-being.

•Demonstrable experience of using community resources, family and natural networks of support to create sustainable packages of care for individuals and help prevent, reduce or delay the need for statutory services.

•A range of post registration experience including in a health and social care setting

•Demonstrable experience of delegation to qualified/unqualified staff

•Ability to put patient care at the heart of the work you do to improve health care outcomes and service delivery

•Strong commitment to deliver the highest standards and quality outcomes possible, always looking for ways to improve and develop to reach your potential

•Encourage others to develop themselves and the service through improvement, innovation and continuous development

•Demonstrable experience of joint working with others, e.g. patients, colleagues in the delivery of high quality healthcare and of building positive working relationships, respecting and valuing others, being helpful and inclusive

•Ability to cope effectively in stressful situations

•Ability to critically analyse and plan

It is desirable that the post holder has:

•Experience of working in a community setting.

•Demonstrable experience of working with long term conditions with older people

•Experience of working in a neurological or orthopaedic setting

•Experience of working in a rehabilitation setting

•Demonstrable experience of working with staff in social, especially in relation to older adults and adults with physical disabilities.

•Knowledge of the National Framework for NHS Continuing Healthcare funding.


It is essential that the post holder has:

•A strong ability to communicate effectively in English in both verbal and written formats

•Excellent verbal and written communication skills

•Sound IT skills and competent in using a computer

•Ability to demonstrate sound clinical knowledge

•Ability to prioritise workload

•Ability to cope effectively in stressful situations

•Ability to critically analyse and plan

•Specialist knowledge of quality models
