Creative Support is a national not-for-profit organisation which provides care and support to people with a variety of needs such as learning disabilities, autism spectrum conditions, mental health needs, and older people.
We are seeking an experienced practitioner to undertake the interesting and responsible role of Support Co-ordinator for Supported Living and Outreach services in Birmingham for people with a learning disability, autism spectrum conditions and mental health needs.
In our supported living services, individuals with a learning disability are supported within their own homes to live as independently and as safely as possible. Each individual has their own tenancy with either Creative Support, a registered social care landlord, housing association or local authority.
Our Birmingham Outreach Service supports adults with a learning disability to access the community and to help support individuals to further their independence, participate in meaningful activities and make the most of opportunities in the local community.
As Support Coordinator you will assist the Birmingham Team Leader and Area Manager in the operational management of high quality supported living and outreach services that are responsive to the needs and preferences of tenants and their families. You will ensure that all contract requirements and expectations are fully met. You will help to organise and provide person centred support for tenants which enables them to enjoy a fulfilling and valued life, to participate in the community and develop their abilities as fully as possible. In addition to this you will support and supervise staff, and deploy staff resources effectively to meet the needs of the service. You will liaise effectively with other involved professionals to provide a consistent and coordinated service.
You will be confident and experienced in supporting people with learning disabilities. You must be observant and emotionally intelligent and have excellent leadership skills. Relevant experience and qualifications (such as QCF 3/5) are required, as are excellent communication skills and a good general standard of education.
