Compass Associates are proud to be working a valued nationwide client to recruit for a social worker for their dedicated low secure hospital for Men and Women suffering with mental health issues and learning disabilities based in Market Drayton. The successful candidate will need to work with the multidisciplinary team to provide professional social work practice and support across an allocated pathway. The post holder will be responsible for the delivery of social work services, ensuring that the needs of individual patients are met, provide advice to staff responsible for service provision, take a lead role in discharge planning with external paties and support with safeguarding and datix investigation and audit where appropriate.

The Candidate - Social Worker (Mental Health & Learning Disabilities Hospital)
This role is an exciting opportunity for an experienced Social worker seeking a new opportunity and looking to join a nationwide organisation. For this role your responsibilities will consist of but wont be limited to providing social work input to the assessment, treatment and rehabilitation of patients within the service; undertake pre-admission assessments as part of the pathway team and particularly where social care issues appear prominent; compiling a comprehensive social history report on all patients during the Intake Evaluation and contribute to an individualised clinical formulation in the initial CPA meeting; contribute to the multidisciplinary care plan through a combination of (i) specific and unique social work inputs, (ii) co- working with colleagues to achieve identified patient outcomes and (iii) individual and/or group counselling/therapy in areas of competence, as appropriate; contribute to ongoing risk assessment, risk management and risk reduction in relation to (i) individual patients and (ii) the unit's milieu and integrity; to act as Care Programme Co-ordinator for identified patients within the service; contributing to the unit's clinical programme and maintenance of the therapeutic milieu as appropriate and agreed; identify, research, assess and advise the multidisciplinary team and patient on aftercare services and rehabilitation needs as patients move through the care pathway; contributing to rehabilitation planning for patients moving through the care pathway to less restrictive environments; ensuring that patients are aware of their rights to Mental Health Review Tribunals and to make appropriate arrangements for their legal representation; ensuring that links are initiated and maintained by Social Workers representing the responsible Local Authority and that attendance at CPA meetings is facilitated and reports provided for MHRT's.

This role is based in Stafford and is commutable from Stoke on Trent, Cannock, Telford, Walsall, Shrewsbury and Market Drayton
