Shared Lives Development Worker

PSS Midlands provides Shared Lives services to individuals with learning disabilities, older people who experience mental health problems as well as to people with physical and sensory impairment.
You will have primary responsibility for the promotion, assessment and recruitment of prospective self-employed Shared Lives Carers within Wolverhampton. In addition to this, you will also be responsible for the day to day support of existing service users assessing their needs and generating outcome focused support plans and risk assessments.
You will have a sound understanding of person centred approaches in care and support planning. You will have experience of working positively with health and social care professionals to deliver positive outcomes and hold a Level 3 Diploma in Health and Social Care (or comparable), or be working towards this.
You will have excellent verbal and written communication skills, and have the ability to motivate and engage Shared Lives carers and people who use the Shared Lives service to promote positive outcomes, quality and continuous improvement.
The successful applicants will be required to travel on a regular basis therefore a full driving licence and use of a car is also essential.
