Job Title: Team Leader (Homes)

Responsible to: Operations Manager/Home Manager

Main Purpose of Role:
Is to ensure service delivery to clients is person-centered, safe and compliant with CQC and the policies and procedures.

To ensure all staff provide and maintain the highest levels of care and service in accordance with the Company's vision, policies and procedures.

Key Responsibilities:
*To support the Manager in the running of the home, undertaking all responsibilities in their absence.
*Set-up, implement and review client care and support plans.
*Lead the shift, ensuring staff give support that follows individual's care plans and daily support plans.
*Ensure all associated documentation is completed in a timely way including Mental Capacity Assessments, client risk assessments, medication plans and medication record sheets etc.
*Undertake Support worker supervision including write-up of notes.
*Undertake observation of Support worker performance including write-up of notes.
*Ensure new starter team/client induction and monitor to sign-off.
*Hold regular team meetings including preparation of meeting notes/actions.
*Hold and/or participate in meetings with clients, their families or representatives, relevant professionals etc.
*Undertake regular audits of the service.
*Support the manager in compiling a four week rota in accordance with the service need, working on shift where required.
*Participate in the on-call rota (which will include some evenings and weekends).

Other General Responsibilities Include:
*To act as a positive role model at all times
*To be flexible and responsive at all time to meet the changing needs of the service and the service users
*Where client finances are handled by us, ensure that all financial management protocols and reporting is adhered to.
*To ensure that medication is ordered, booked in and administered according to policy and procedures.
*Ensure medication errors are documented appropriately and a copy of the medication error form is sent to Head Office without delay.
*Report immediately to the Manager any illness of an infectious nature or accident incurred by a service user, self or other member of staff. Complete appropriate documentation and ensure a copy is sent to Head Office.
*To ensure Health and Safety at Work responsibilities are carried out in accordance with Company procedures.
*Support clients with regards to decision making and lifestyle choices enabling them to take control of their own lives, increasing independence.
*Ensure all personal care tasks are carried out in line with the needs, wishes and preferences of the individual, following the support plan.
*Support people to develop and expand social networks and friendships.
*Develop positive relationships with family and carers/support workers.
*Work in line with the company's Adult Protection and Prevention of Abuse Policy and Whistle Blowing Policy to ensure that people are kept safe at all times.
*At all times to work within equal opportunities policy and procedure.
*Attend training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company requirements.
*To be fully committed to the continuation of personal professional development.
*To work in accordance with the company's mobile phone and IT policies.
*Maintain appropriate confidentially at all times.
*To carry out any other duties as may be deemed necessary, which are consistent with the basic objectives of the post.

Working Hours:
-Full-time, 40 hours per week.
-As support workers work shift patterns over a seven day period it will be necessary to carry supervisory activities accordingly.
-This may include weekends and bank holidays.
