Position: Dementia Support Worker Location: Telford Contract type: Fixed Term Contract - Until 31st March 2019 Hours: 35 per week Salary: £19,985 actual per annum Closing date: 19 October 2017 Interview date: 25 October 2017 About the role: We are seeking a DSW covering Oswestry and surrounds, at 35 hrs per week. Due to the geographic spread of the service provision the ability to travel independently would be preferable. You will be joining an expanding team and will provide support, information and guidance to people with dementia or their carers and wider family or friends throughout Oswestry. You will be part of our exciting and new ways of working closely with GP practices and Memory service to improve the current journey of people affected by dementia. This service will be person centred as it will be led by conversations with clients as part of their own Living well Plan. We are looking for people who can support creative solutions within this challenging work stream as the Alzheimer’s Society embarks on its new Five Year Strategy. You will be known locally as a Dementia Companion. This service is provided in a range of settings, including the clients home, clinics and in group settings. The service may include signposting and providing referrals to other services. To ensure service users continue to live as they want to within their community you will also work with volunteers who are supporting local communities to be ‘dementia friendly’. About you: Educated to NVQ level 3 or equivalent, you will have excellent time management and, communication skills. You will have proven facilitation skills as well as being a good listener. You will also have a sound understanding of the value of groups. You should have a caring style and an understanding of a person centred approach. You should have relevant experience of record keeping and report writing, assessing the needs of the individual to support and enable them to continue living well with in their community. You should be confident in the use of IT. Experience of working with statutory and voluntary agencies would be an advantage. An understanding of relevant legislation such as the mental capacity act is required. You will be part of a small team in Shropshire, Telford and Wrekin and across a wider Locality making a difference to the lives of people affected by dementia.
