Community Nurse Band 5 or 6 - Birmingham and surrounding. Permanent - Full Time.

Pulse provides safe, effective social care and health support services to people in their own environment. We work as a trusted partner with a variety of organisations to provide 'carepackages’ in the community. These include hospitals and primary care trusts, local authorities, GPs, charities, case-management companies and voluntary organisations as well as private individuals

Pulse support people with a variety of specialist care and nursing needs. Our aim is to make a positive difference to the well-being of individuals and where possible improve their wellbeing. We work with over 200 commissioners nationwide and work in partnership with clients and commissioners to design very specific and personal clinical care plans to enable our clients to live as independently as possible, encompassing their likes, dislikes and preferences wherever possible.

To do this, we require confident and passionate nurses to join our friendly, hard working team within Birmingham.

This role is ideal for someone looking to take on a new challenge, whilst further developing their clinical skills, and is made up of both case management and the development and implementation of clinical care plans directly with clients in the community.

This role is permanent and full time and you will be based at our West Midlands branch. The role will require you to manage your own clinical case load of clients and will be a mixture of working within the community and within our Pulse Community Healthcare office in Birmingham city centre.

Your role will focus on ensuring we are providing high quality and safe clinical practice at all times with the full support of a central clinical governance team.This will include:

  • Producing and implementing care plans,

  • Policy writing (within scope of practice)

  • Auditing of clinical documentation

  • Reviewing and monitoring the clinical aspects of care packages

  • Competency assessments of care workers

  • Training within your scope of practice

  • Liaising with other healthcare professionals when necessary.

  • Maintaining relationships with other professionals.

Although the job provides consistency within your case load, the role is also diverse and varied, allowing you to grow and develop in a safe environment. As an ideal candidate for this role, the following is essential -

  • A Registered General Nurse (Valid NMC PIN is essential)

  • Have good experience within an ITU or HDU environment

  • Have relevant experience of working with tracheostomies and ventilators

  • Be a car owner and able to drive due to level of community work

  • You will be passionate about delivering the highest standard of clinical care, whilst maintaining the clients rights towards choice and independence wherever possible

  • Be adaptable and versatile in your approach to work

  • Be an excellent communicator with both clients, and within your supported teams
