Role: Community Care Worker - Adults and Children
Location: Wolverhampton and Dudley

Working Patterns: Days, Evenings, Weekends, Holidays (some nights).
Part time and full tiem hours available.

This is an ideal chance for people to join our growing and exciting home care services. We provide a provision to adults and children with learning and physical disabilities who live in the family home.
By its nature home care requires Community Care Workers to travel from place to place supporting individuals to remain as independent as possible within their own homes and out in the community.
We are looking for somebody who is flexible, reliable, enthusiastic, dedicated and want to make a difference in children’s lives. You will have responsibility for providing a person centred service to a caseload of customers within the community whilst demonstrating a commitment to the service co-production agenda. You will be providing a full range of care and support, when required administration of medication, manual handling and toileting.
You will be available at a time to suit the family, child/young person with a particular emphasis on evenings, weekends and holidays. You will be required to evidence how disabled children and young people are encouraged and supported to take part in inclusive mainstream universal activities and socialise with their peers. You will encourage and support disabled people to develop independent living skills such as travelling, confidence with money and social interaction.
The role involves providing a high quality frontline care service to adults in their own homes. You will have responsibility for providing a person centered service to a caseload of customers in the community.
You should hold a Level 2 (or working towards) Caring for Children and Young People, Health and Social Care or equivalent. You will be committed to all training commensurate to this post and demonstrate competency and understanding to be a safe practitioner.
You must have effective communication skills, be creative and of a playful character whilst being resilient and engaging with children who may have a difficult background. Your role will be instrumental in offering families, children and young people a break from their main routine and caring duties.

• Support with domestic/household tasks
• Completion of customer records which recognise the individual their achievements
• Actively supporting and promoting fundraising initiatives
• Social engagement and involvement
• Support with personal health and wellbeing
• Promoting customer independence
• Liaising and engaging with families, stakeholders, internal and external partners
• Supporting the Coordinator with other duties to ensure the smooth running of the service
• Completion of audits as required and identification of required actions- KPI’s, financial audits, health and safety, medication audits
• Review, contribute and develop customer assessments and reviews which identify risk, achievements and outcomes in a person centred ethos
